Eligibility Requirements
To be eligible for enrollment in the program:
- Household income and family size must meet the established DSS and CDE income guidelines.
- Need for services must be present.
- The child must be 6 weeks old to Pre-Kindergarten age.
Enrollment Application Process
You will need to provide the child’s proof of birth date, proof of your household income, the child’s immunization record & proof of your current employment and/or school or training schedule, or self certify that you are seeking employment. Upon acceptance into the program, physical & dental examinations will be required & a Health History must be provided.
